If you are a new patient, we will send you a email with a link to fill out your "New Patient Paperwork" online. If you don't get a chance to complete the paperwork before you visit, don't worry! You can complete it in the office.
We see and treat patients for a wide range of conditions and diagnoses. Please visit our Treated Conditions page for an extensive list of conditions and diagnoses we see patients for.
We are a non-participating provider with Medicare which means we will collect at the time of service, and bill Medicare on your behalf. Medicare will send you an explanation of benefits, and reimbursement of their allowable amounts.
Patients with verified claims will not be financially responsible for services related to your worker’s compensation injury. If your claim cannot be verified, you will be expected to pay for services in full at the time of service.
Patients are requested to make payment in full at the time of service. However, if you are not able to meet the full payment, we offer flexible payment plans.
We accept most insurance plans. While you are having your complimentary consultation with the doctor, our front office staff will call to verify your insurance co-payments and deductibles.
